In designing and implementing policies, Dr. Pomeroy has a checklist of solutions that are implementable now, including the use of a social vulnerability index, screening for and integrating SDOH data into electronic medical records, linking to community resource directories, social assistance and reimbursement in payment models, and workforce diversity in the people providing healthcare.
